Many people who smoke or vape worry that this might stop them from donating a kidney, but being a smoker doesn’t automatically mean you cannot donate. What it does mean is that your lifestyle and health will be looked at particularly carefully during the donor assessment. 

Kidney donation is a major operation and doctors want to make sure the process is as safe as possible for you. Continued cigarette smoking does increase the risks of donating both in the short-term and long-term, and many people have taken donation as an opportunity to stop smoking, and to get support to do so. 

 

NHS guidelines on smoking and kidney donation 

The NHS does not automatically rule out smokers from becoming living kidney donors. Instead, every potential donor is assessed individually. Your medical history, lifestyle, and current health will all be taken into account. 

This means that while smoking does increase the risks during kidney donation, it doesn’t stop you from being considered. What matters most is your overall health and whether the medical team believes donation will be safe for you – both in the short and long term. 

 

Why smoking matters in the kidney donation process 

When you donate a kidney, you are having a significant surgery and living with one kidney afterwards. Because smoking can affect circulation, blood pressure, and recovery, it becomes an important factor in the medical assessment. 

Some of the areas doctors consider include: 

  • Whether smoking might increase the chance of complications during surgery 
  • The long-term function of the remaining kidney 

These points are not meant to stop you from donating. They’re part of the process to make sure donation is safe for you and that your remaining kidney will continue to keep you healthy. 

 

Will you be asked to quit smoking? 

In many cases, transplant centres encourage or ask donors to stop smoking before surgery. This is because quitting, even for a short period, can lower the risks linked to the operation and help your body recover more smoothly. 

How long you might be asked to quit smoking for, both before and after the donation, can vary, though ideally you should stop smoking as far in advance of the surgery as possible. Some centres may set a minimum time without smoking, while others will give general advice. The important thing to remember is that this request is about your safety. 

 

Support to help you stop smoking or vaping 

If you’re thinking about quitting, the NHS provides free services to help people stop smoking or vaping. These include: 

  • Specialist ‘stop smoking’ advisors who can give you tailored support 
  • Treatments such as nicotine replacement therapy (patches, lozenges, or sprays) 
  • Prescription options that reduce cravings 
  • Access to local support groups, phone helplines, and online tools 

Even if you’ve tried to quit before, using structured support can make a big difference. Stopping smoking is not only good for donation, but also benefits your long-term health. 

 

What if you can’t quit? 

Not everyone finds it easy to give up smoking. If you’re unable to stop, donation may still be possible depending on your overall health. Your assessment will include tests and checks to see whether it’s safe for you to proceed. 

In this situation, doctors will weigh up the risks and talk them through with you. The aim is always to make sure that donation does not put your health at risk now or in the future.
